Somerset Supportive Housing Program

How Our Evidence-based Approach Can Help You Reach Your Housing Goals

People with psychiatric disabilities have the right to live in a home of their own. However, they often face barriers that prevent them from having a stable living environment. The Supportive Housing Program of Cornerstone Community Services provides housing support services designed to help adults with a serious mental illness (SMI) or co-occurring disorder of substance abuse and mental illness. Our program offers voluntary, flexible supports to help one find, choose, and keep housing that is decent, safe, and affordable.

The focus of the program is to teach individuals skills they will need to live independently and to provide case management services to link them with community resources. The program is designed to assist persons to live independently through skill development and support. Our program staff assist individuals to develop attitudes and habits necessary to prevent re-hospitalization or homelessness and to encourage successful community integration.

Our goal for program participants is to become responsible tenants through support and instruction in the areas of:

  • Independent living skills
  • Budgeting
  • Mental health recovery
  • Accessing housing resources and subsidies
  • Community integration

Housing Support can be provided independently or in combination with Permanent Supportive Housing services through the Community Action Partnership of Somerset County (Tableland Services, Inc.) who provides additional housing subsidies and funding. These services are funded by HealthChoices Reinvestment funding provided by the Behavioral Health Services of Somerset and Bedford Counties, Inc. (BHSSBC) and funding provided through the Bedford-Somerset DBHS. There is no cost to individuals receiving these services.
